Mental Health Case Manager - Available Now! December 2025

Mental Health Case Manager 

New Open Positions – December 2025- Van Wert, Delphos & Celina locations

Service Locations: Van Wert, Delphos, Paulding and Celina offices
Status: Full time and Part Time Available 

Responsible for building and maintaining therapeutic relationships and providing basic outreach to a caseload of clients who have a history of severe and persistent mental illness and/or other drug abuse or dependence.

Coordinates Individual Service Plan (treatment plan) for clients. Assists and supports the client in crisis situations. Offers support including education and consultation to family or significant others. Aids the clients in symptom monitoring and self-management of symptoms to facilitate appropriate adaptation. Assists in the development of skills for recovery and successful community living.

Assists and supports the client in obtaining and utilizing the essential community resources as well as the involvement in positive relationships and activities within the community.

A Bachelor or Associates degree in social work or a related degree (psychology, sociology, education, criminal justice), and/OR two years of  work experience in a related field.

A complete benefits package is available, and salary will be commensurate with education and  experience.

No weekends or on-call responsibilities are required in this position.  

Outpatient Therapist

New Open Positions – December 2025- Delphos & Celina locations

Provides Diagnostic Assessments and Individual, Group and Family therapy to a diverse population.   

Applicants must hold licensure as one of the following:  LSW, LISW, LPC, LPCC, LCDC III, LICDC,  Psy. D., Phd., or the have the ability to immediately pursue such licensure.

No weekends or on-call responsibilities are required in this position. 

All CEUs and Licensure Renewal fees are covered.

Addictions Case Manager

 

This position assists individuals who are diagnosed with substance use disorders.  The case manager works closely with our local courts and probation officers to maximize positive outcomes.  This position supports the client in their recovery plan and links them with needed resources such as peer support, housing, employment, healthcare, entitlement programs, childcare etc. Assists in the development of skills for recovery and successful community living. Assists and supports the client in obtaining and utilizing the essential community resources as well as the involvement in positive relationships and activities within the community.

This position requires no evenings, no weekends and no on-call responsibilities.  

A Bachelors  or Associates degree in social work or a related degree (psychology, sociology), and/OR two years of  work experience in a related field is required.

School Based Mental Health Therapist - Available Now! December 2025

School Based Mental Health Therapist – Position Available in Delphos

This outpatient therapy position is embedded within the school buildings to provide on site mental health services to students ages 5-19.  Diagnostic Assessments will be provided as well as outpatient therapy services.  Prevention services are also offered as well as staff consultation.  

Applicants must hold licensure as one of the following:  LSW, LISW, LPC, LPCC,   Psy. D., Phd., or the have the ability to immediately pursue such licensure.

Peer Support Worker -Available now! December 2025

Duties include meeting with clients recovering from addiction in the community and sober living settings.  Peer supporters come alongside clients and support and assist them on their path to recovery while expanding their sober support network. 

  • Assist individuals address barriers to recovery 
  • Provide support to maintain recovery and reduce relapse
  • Conduct Outreach in the community 
  • Teach skills to effectively navigate the health care system

Must have valid driver’s license.  Must hold or be willing to pursue a
Ohio Peer Recovery Support Certificate.

Clerical/Receptionist Available Now! January 2026
Summary
This position is responsible for answering incoming telephone calls, facilitation of the smooth flow of clients through the waiting room to their appointments, assisting with new client paperwork, scheduling client appointments, computer documentation, filing of client records.  This position will also be involved in typing, filing, copying and interoffice communication other clerical duties of this kind. Part-time employment, 24-28 hours per week. 8am – 5pm and 11am – 8pm, hours range.
We are primarily seeking a part-time team member, but may consider full-time arrangements based on experience, availability, and fit. 
Experience
High School Diploma/GED. Prefer experience with Microsoft Excel. Knowledge of Medicaid and private insurance is appreciated, but not required.
