Call our intake phone number in Van Wert: 419-238-3434 or Paulding: 419-399-3636 (expect to spend 5 minutes on this call).
New open access schedule: If your paperwork is complete come to our offices at these days and times for your initial diagnostic assessment (clients served on a first come, first serve basis). Paperwork can be picked up at any of our locations or complete and submit the online New Patient forms.
Van Wert office location:
Monday: 12:30pm, 1:30pm and 6:30pm
Wednesday: 1:30pm, 3:30pm, and 6:30pm
Thursday: 9:30am and 10:30am
Delphos office location:
Monday: 9:30am
Thursday: 2:30pm
Paulding office location:
Monday: 2:30pm
Wednesday: 9:30am and 3:30pm
Friday: 12:30pm

Photo I.D
Medical insurance card
Proof of income
Name of primary care physician
The assessment involves a confidential meeting with a clinical professional to arrive at a diagnosis and plan a course of treatment. This initial meeting generally lasts about two (2) hours.
What to need to bring if the appointment is for a child:
(Guardian must accompany the child)
Certified documentation and/or verification of guardianship
Social security card or birth certificate
Medical insurance card
Name of referring physician/clinic/school
