Diagnostic Assessment and Intake
Intakes/Diagnostic Assessments are face-to-face interventions for the purpose/goal of assisting the client in dealing with their crisis and/or illness. A clinical evaluation identifies the nature and extent of the presenting problem, including substance abuse, depression, anxiety and behavioral issues.
The assessment includes: discussion about the client’s strengths, needs, abilities, and preferences, as stated by the client; discussion about the various therapeutic techniques available to meet the needs and preferences, a projected length of treatment including other diagnostic exams or techniques recommended; discussion and resolution of any special needs of the client, such as, physical impairments, etc. and documentation that these needs have been addressed and met.
